Appointment requests

The appointment form asks only for your name, contact number, requested service and an optional scheduling note. The website does not save or transmit this form to Tesla Imaging. It prepares a WhatsApp message on your device, which you can review before sending.

Do not submit medical information

Do not enter symptoms, diagnoses, prescriptions, report details, test results or other sensitive medical information into the website form or an unverified public message. Contact our team by telephone for appropriate instructions. Patient and clinical records are handled separately through authorised patient-care systems and processes.

How we use contact information

When you choose to send a message or call us, authorised staff may use the information you provide to respond to your enquiry and coordinate an appointment. WhatsApp and other linked services operate under their own privacy terms.
